Students must complete the cover letter attached below when submitting community service hours to their counselor. Each and every time students submit hours to their counselor, students must complete the cover letter and attach it to the front of the documentation. The documentation can be the log below (signed by the supervisor) or another form of proof that was provided by the non-profit organization.


Students, not parents, are to submit the hours to their counselor. Students must submit all of their hours to their counselor by December 13th of their senior year.
Instructions for students:
Please print the Cover Letter below, complete it, and attach it to documentation/proof of your completed hours. Please then submit all completed documentation in person to your counselor.

Remember to complete all information on the forms, including your name, ID number, and total amount of hours you are submitting (please add them).
